Preparing for your Visit

If you are a new patient, please download and complete the New Patient Packet prior to your appointment. The questionnaire takes time to complete and is best done prior to your visit. If you are unable to complete the forms prior to your visit, please arrive 30 minutes early so that you can complete them in person.

Also for your initial office visit, please bring the following:

  • Photo ID, for example driver’s license
  • Insurance cards
  • List of all medications – prescription, herbal and over-the-counter drugs that you are currently taking, including dosages; or the prescription bottles themselves
  • Insurance co-payment or deductible if required by your insurance plan. Co-payments as well as any fee-for-service and non-covered service fees are due and payable at the time of service.


If you find that you are unable to keep your appointment at the scheduled date and time, please notify us as soon as possible and speak directly to office personnel at the front desk. Messages left on the voice machine cannot be guaranteed as received. We request at least a 24-hour notice for cancellations.

Overnight Sleep Study Instructions

Daytime Preparations:

  • After 12 noon on the day of the sleep study, do not consume any products with caffeine, alcohol and/or stimulants, including chocolate.
  • Avoid taking naps
  • Skin must be free of lotions and any other type of moisturizer; remove all nail polish.
  • If you normally shave your face in the morning, please shave that afternoon or evening. If you have a beard, mustache or goatee, you do not need to shave it off. 
  • Hair must be free of gels, conditioners and hair spray. Wash your hair on the day of your study.
  • If you need transportation, please make arrangements prior to your scheduled study.
Night-Time Instructions:
  • Please be on time for your scheduled study.
  • No tobacco products may be used during the course of the sleep study.
  • Only patients are allowed in the building during the sleep study. For privacy and security reasons, family and friends are not allowed to stay the night unless they are the designated caregiver.
  • You will be able to leave the facility at 6:00 AM. If someone is picking you up, please have him/her arrive no later than 6:30 AM.

Items to Bring
  • All patients are required to sleep in regular pajamas, not silk pajamas, T-shirts and shorts, or sweats. Nude sleeping is prohibited Please wear loose, comfortable pajamas. For women, pajamas are preferred in lieu of nightgowns.
  • Bring all of your night-time medications with you. Upon arrival, take medications as you normally would unless otherwise instructed by your physician.
